		<description>I have taken both H1N1 and seasonal flu immunizations.  Unfortunately the cards that I received from the  agencies who provided the vaccinations are not acceptable according to vendormate.  The cards have the location of the facility, my name, the date of the immunization, and the type vaccine issued.
What esle do you need?</description>

 I need to know what hospitals does vendor mate cover in the states of Michigan, Ohio, Indiana, Kentucky &amp; Pennsylvania.  

Do I have to take and pay for these course again, each year or this a one time event?

With covering 5 different states and so many different vendor companies like yourself, one could go broke or spend many hours managing all the requirements one has to complete to keep in compliance with all the different hospital I deal with.</description>
